- After your registration is approved, you are eligible for reimbursement for the repair of your broken camera lens based on the specified amount of your original purchase (up to $100). Repair must be performed by a Samsung Authorized Repair Center location.
- Case-Mate does not replace phones, this program only covers your camera lens repair for up to one year.

Before filling out the registration form, please have the following available:- 15-digit Phone IMEI Number — to locate, dial *#06# on your phone or click here to find in your settings
- UPC Item Number — located on the barcode on the back of your item packaging (12 numbers)
- Your store or online order receipt
